Clinician App Improves Care and Makes Graham Healthcare Group a Top Employer

Company Background

As an innovator in an evolving healthcare world, Graham Healthcare Group has been designing business and technology solutions to drive better care, outcomes, and productivity within its own home health and hospice companies for over 20 years.

Graham Healthcare Group offers home health and hospice services in Michigan, Illinois and Pennsylvania under multiple brands including Residential Healthcare Group, Allegheny Health Network Healthcare@Home, and Mary Free Bed at Home. Graham Healthcare Group has successfully integrated their technology solutions across brands and the healthcare continuum.

Challenge

Technology innovation is a core principle for Graham Healthcare Group. In looking for ways to improve patient care and help make their clinicians more efficient, they decided an app could help do this. They were already working on a patient mobile app, called My Care Central, but wanted just as much focus on a clinician app that could streamline care. They brought their requirements to SharpQuest to partner with them to build the clinician app and a platform to support it.

Results

Improved care processes yielding an average 4.9/5 patient rating

Improved clinician satisfaction with easier workflow and positive feedbacks

Reduced costs from printed materials

When you look at the dashboard and you’re seeing the numbers growing, and our feedback is so high from the app, it’s just been a really great story to tell.

SharpQuest made it all work together seamlessly. And they built us great dashboards and analytics that are really important to our company to understand how to build upon the success that we’re having.

Solution

The SharpQuest team built a patient and caregiver facing mobile app called My Care Central, which would allow patients to communicate with clinicians and nurses, see their appointments, medications, and much more. To truly add value, this also required having an app for clinicians in the field.

SharpQuest built The Clinician Hub, which is a progressive web app (PWA) that is used by clinicians on their Android tablets. The app allows clinicians to easily onboard patients and caregivers to use the mobile app—which is critical for elderly patients. Clinicians can also send secure messages to patients and caregivers, conduct virtual visits, recommend educational videos and reading material, track supplies, and see feedback from patients after visits.

Spencer added that they will continue to add value to the Clinician Hub. “It’s the area where clinicians are connecting
and seeing what’s going on with their patients. We definitely see a lot more use cases for the clinicians to help in the
future and we’re really excited to continue to work with SharpQuest on that.”

SharpQuest also built a platform to bring all the data together for Graham Healthcare leadership to manage their business. They created an administrator dashboard to review patient feedback and app data at branch and corporate levels. Spencer commented, “SharpQuest made it all work together seamlessly. And they built us great dashboards and analytics that are really important to our company to understand how to build upon the success that we’re having.”

Outcomes

Graham Healthcare’s clinicians have been very pleased with the Clinician Hub. And they really like seeing the feedback from patients. The overall patient rating after clinician visits is 4.9/5. The comments they receive from patients has been very encouraging and they share it regularly with their staff for motivation. “It makes the clinicians feel so good when they get a five-star rating, which I see every day,” notes Spencer. “It just lightens everybody up. Everyone gets so excited because that is what the goal is, to always deliver top care.”

The technology streamlines patient/clinician communication. Spencer added, “The clinicians are really loving the app because it has improved communication. Phone calls are hard because clinicians are in the field taking care of patients. With messages in the app, it’s a lot easier. They can answer back after a visit and anything urgent is handled by office staff.” Spencer also believes this is improving clinician retention. “The app has really improved clinician satisfaction. I also think it shows that we’re innovative and that we’re investing in technology and solutions for our clinicians.”

The app is also saving Graham money from not having to print patient materials. They used to print a “Start of Care” book as well as provide printed materials for education. All this information is now provided digitally within the app and clinicians can push relevant materials directly to their patients.

SharpQuest ensured the app was “white labeled” so it could easily extend out to various Graham Healthcare Group brands, with all the data coming back to headquarters. “It’s a unique experience for each patient and our clinicians are loving it,” notes Spencer. “And when you look at the dashboard and you’re seeing the numbers growing, and our feedback is so high from the app, it’s just been a really great story to tell.”
